Some pairs use a bid of 5NT (not preceded by 4NT) when the partnership has bid more than one suit to indicate that the values for a slam are present but doubt remains about strain.
In general it is a good idea to establish the trump suit before embarking on a slam hunt, so the convention usually occurs only in highly competitive auctions where the opposing bidding has prevented the customary exploration.
